常態行動心理学的尊厳死について♪

本日のテーマは『尊厳死』について。 私自身ホスピスの患者さんのmentalTherapyを何度かさせて戴いた中で『尊厳死』について深くおもいを馳せる機会を頂戴しました。 読んで字の如く『尊厳死』とは『尊く厳かに死を迎える』と書きます。 日本と海外とではホスピスの概念が大きく異なります。 日本のホスピスは残された命をなるべく延命させようという考えの元、自由度がかなり制限を受けています。 これはもしも療養中に患者が死亡した際の責任の所在を病院側に求められる危険性回避の為です。 言わば保身に走っている訳で患者第一を貫いてはいません。 翻り海外ではきちんとした誓約書を交わした上で自由度を極限まで高めそれにより延命を図って居ます。 私自身が嘗て扱った案件の話ですがホスピス患者がチャルメラの音に誘われて拉麺を食べたいと申され病院に内緒で食べさせてあげた事があります。 この患者は内蔵疾患で塩分制限を掛けられ何時も薄味の食事ばかりを食べさせられてました。 勿論病院側にはお叱りを受けましたがその後の検査で驚くべき結果が出ました。 拉麺を食べる前と食べた後では明らかに拉麺摂取後の方が数値が正常値に近くなっていたのです。 病院側は奇跡だと言われてましたが生憎精神心理学的には極めて当たり前の結果だと言わねばなりません。 濃い味嗜好の方にとって薄味はストレス以外の何物でもありません。 そのストレスが血中に毒素を垂れ流し結果的に数値をあげていたのです。 されど数年ぶりに濃い味の拉麺を食べる事が出来た為にストレス値が下がり血中に垂れ流されていた毒素は無くなりそれが検査結果を良好にしただけです。 それからも私はたまに差し入れをして差し上げそのお陰で数値は安定し当初予定されていた命の期限が大幅に延長され最後は老衰で亡くなられ検死ではガン細胞は見付からなかったそうです。 これは細胞学的にも免疫学的にも充分説明のつく話です。 加齢と共に細胞再生能力は低下し弱くなります。 其処に充足感が加わると異物を取り除く力は増加します。 結果的に臨終の際にはガン細胞は死滅していてもおかしくはありません。 現代の日本のホスピスは当時と比べ大分改善されているようですがまだまだ『尊厳死』の考え方は欧米と比べ随分精神的に立ち後れて居ます。 その遅れこそが医療全体の遅滞を生んでいる事をそしてそれがホスピス患者を苦しめている事を医療従事者は痛感し改善すべきと考えます。 勿論末端の医療従事者は皆、その事に気づいています。 されど病院経営者や厚労省は全く気づいていない。 或いは気づいていながら気づかないふりをしているのかもしれません。 私はこれからも一人の医療従事者として私なりのやり方で『尊厳死』について考え行動をしていく所存。 Today's theme is about "death with dignity".
